Assumption of Risk and Release of Liability In consideration of my child’s participation in activities organized by FutsALL Academy, I, the undersigned parent or legal guardian, acknowledge and agree to the following: Inherent Risks Participation in futsal and related activities involves risks of injury, including slips, falls, collisions, and other accidents. While rules, proper equipment, and supervision reduce risks, serious injury remains possible. Assumption of Risks I knowingly and voluntarily accept these risks on behalf of my child. Waiver and Release I, for myself, my child, and our representatives, release and hold harmless FutsALL Academy, its coaches, officials, staff, volunteers, sponsors, and facility owners from any liability related to injury, disability, death, or property damage arising from participation, whether caused by negligence or otherwise, to the maximum extent allowed by law. Medical Authorization In an emergency, I authorize FutsALL Academy staff to obtain medical treatment for my child. I understand I am financially responsible for such treatment. Concussion and Head Injury Policy I understand the risks of concussions in sports and agree to the following: A concussion is a brain injury caused by a bump, blow, or jolt to the head or body. If my child is suspected of having a concussion during practice or games, they will be removed immediately and may not return until cleared in writing by a licensed healthcare provider. This policy is designed to protect my child’s health and safety. For more information: cdc.gov/headsup Registration Agreement and Sportsmanship Pledge By enrolling my child in FutsALL Academy, I agree to the following terms: Fees and Registration All registration fees are non-refundable. A team spot is only confirmed once full payment is received. Playing time and game schedules are not guaranteed. Sportsmanship FutsALL Academy promotes fun, fitness, and respect. Disrespect, negative comments, or unsportsmanlike conduct will not be tolerated. Players, teams, and parents may face suspension or dismissal for misconduct, without refund. Use of Likeness I grant FutsALL Academy permission to use photos/videos of my child for promotional purposes. My child’s name or identity will not be disclosed without consent. Communication I consent to operational emails and texts from FutsALL Academy, with the option to opt out of marketing at any time.
